As a part of therapy and/or consultation services, and in accordance with best practices, information will be obtained using the various forms listed below.

Some of these forms ask questions about you, your family, and why you are seeking services. The forms also provide information about Carmel Mindfulness Project’s policies and practices, and to obtain various consents necessary for the provision of services. It is important that you understand this information, and clients will be asked to submit signed copies of certain required forms to initiate our work together.

It is possible for potential clients to review, download, print, and complete the forms and bring a copy to their first session. This will save a lot of time during the initial session. If you cannot print these forms, have questions, or would like to talk about them in person, I have copies of the forms in the office and will be happy answer any questions and assist you in understanding and completing them in session.

All necessary forms will be reviewed with you and the links below are provided as a reference for you to print out or read on-line. Other forms may be required as services proceed.

Note: The Client Intake Form is very broad and asks many personal questions that may or may not be necessary to initiate services. It is not necessary to have this completed before the first session. Most people are more comfortable disclosing some information on this form in person.
